This Agreement (“Agreement”) explains how medical and nutritional services are delivered through Measured and what you must understand before receiving care. By electronically signing — or by clicking a box labeled “I agree” — you acknowledge that you have read, understood, and accepted every provision below.
1. Parties and Scope of Care
Measured operates the technology platform and provides non-clinical support. Clinical care is provided by independent medical groups as shown in the table.

You may join either program or both, and you may discontinue at any time under §12.
2A. Platform Experience and Technology
When you become a patient, you will gain access to Measured’s mobile or desktop app (“Platform”). The Platform includes personalized content, goal tracking, scheduling tools, and secure access to your care history. It also connects you to clinical and non-clinical support staff, including coaches who may not be licensed providers.
3. Telehealth Consent
Telehealth Definition. Telehealth involves secure electronic transmission of medical information using video, audio, or asynchronous messaging technologies.
Benefits. Telehealth makes specialized care more convenient and may reduce travel time.
Risks. Technical failures could delay evaluation, and data security measures could fail despite encryption.
Alternatives. You may seek in-person care from traditional clinics. If you do not consent to telehealth, you may not be eligible for Measured programs.
Right to Withdraw. You may revoke telehealth consent at any time by emailing careteam@trymeasured.com; such revocation may limit your ability to continue in the program.
Recording of Visits. Some telehealth visits may be recorded for internal quality improvement. You may decline recording by notifying your provider at the start of any visit.
3A. Providers and Credentials
Your provider may be a physician, nurse practitioner, or physician assistant. Exact titles may vary by state. Your provider will identify themselves at the beginning of care, and you may request access to their credentials or bios at any time. You agree that your provider may not be a physician for every visit.
4. Medication Information – GLP-1 & Oral Agents
Measured prescribes several evidence-based medications for chronic weight management. These include FDA-approved GLP-1 receptor agonists such as semaglutide (Wegovy / Ozempic) and tirzepatide (Zepbound / Mounjaro), as well as orals (e.g., naltrexone / bupropion, metformin, and topiramate) and, when clinically appropriate, compounded formulations including liraglutide. All prescriptions are issued at the independent discretion of your treating provider, who will weigh contraindications, insurance coverage, and lab results before initiating or changing therapy.
GLP-1 class medications share a common side-effect profile that may include nausea, vomiting, diarrhea, constipation, and injection-site reactions. More serious but less frequent risks include pancreatitis, gallbladder disease, and — based on rodent studies — thyroid C-cell tumors; oral agents have their own risks such as CNS effects or lactic acidosis (rare). You should read the FDA Medication Guide for any drug you receive and report severe or persistent adverse events to your provider immediately or seek emergency care when indicated.
Disclaimer on Compounded Medications. Compounded formulations are prepared by third-party pharmacies and are not reviewed or approved by the FDA. Your provider may prescribe compounded medications when commercially manufactured alternatives are unavailable or medically inappropriate.
5. Laboratory Monitoring and Adherence Requirements
Initial labs (CMP, HbA1c, lipid panel, and TSH) must be completed within one (1) month of program start. Follow-up labs occur every six to twelve months. Failure to complete required labs may result in prescription holds or program dismissal. If your insurance denies a lab claim, you are responsible for the cash price.
Lab Accuracy Notice. Measured and its affiliated providers use third-party laboratories for diagnostic testing. These labs may return false-positive, false-negative, or inconclusive results, which could impact your care. Neither Measured nor your provider guarantees the performance or accuracy of these tests.
6. Financial Terms
1. Cash-Pay Services
You authorize Measured to charge your saved payment method for membership fees, medication shipments, and lab requisitions. Services are not covered by insurance or Medicare/Medicaid. You agree not to submit claims to any federal payors.
2. Insurance-Covered RD Visits
You assign benefits to Bridge by accepting the Assignment of Benefits and agree to Bridge’s Consent to Treat. You are responsible for any copay or deductible balance.
Patients who elect a stand-alone RD visit will not be charged any membership fee by Measured. All billing for that visit will flow directly through Bridge to your insurer, and you will be responsible only for the normal copay, coinsurance, or deductible determined by your health-plan.
6A. Subscription Terms and Auto-Renewal
If you purchase a subscription, it will renew automatically unless you cancel before the end of your current billing period. By storing a card on file, you authorize Measured to charge it for all recurring and one-time services unless you revoke access.
7. Privacy and Data Sharing
PHI generated during clinical encounters is protected under HIPAA and governed by Bridge’s HIPAA Notice. Non-PHI such as site analytics and marketing preferences is governed by Measured’s Privacy Policy. We share information among clinicians, pharmacies, and labs strictly for treatment, payment, and health-care operations.
Although we use standard encryption and access controls, data transmitted via the internet or stored by third parties may be vulnerable to unauthorized access. You acknowledge this residual risk when using the Platform.

With your other healthcare providers for coordination and treatment
With family members or caregivers involved in your care
As otherwise permitted by our Notice of Privacy Practices and applicable law
8. Expected Outcomes and Limitations
Weight-loss outcomes vary by agent, dose, and individual adherence. Pivotal trials show that adults using once-weekly semaglutide 2.4 mg plus lifestyle counseling lose about 15% of baseline weight after 68 weeks, while tirzepatide 15 mg has produced mean losses up to 22% in 72 weeks; oral and lower-dose options typically yield more modest reductions of 5–10% over similar time frames. Real-world results depend on medication tolerance, dietary choices, physical activity, genetics, and other medical conditions, so no provider at Measured can promise a specific amount or rate of weight loss.
9. Patient Responsibilities
You agree to:
Provide accurate and complete medical history.
Record your weight at least weekly using the in-app tracker.
Take medication as prescribed unless advised otherwise.
Complete required labs within the specified window.
Monitor for side effects and report significant issues within 24 hours.
Keep your mobile device and account credentials secure.
10. No Emergency or Crisis Care
Measured is not equipped to handle emergency situations. If you experience chest pain, severe shortness of breath, vision changes, or other alarming symptoms, call 911 or go to the nearest emergency department immediately.
11. Scheduling and Attendance
We ask that you notify your provider at least 24 hours in advance if you need to cancel or reschedule an appointment. If you miss multiple appointments or are non-responsive over an extended period, you may be removed from the program and lose access to your care team.
For appointments with Registered Dietitians (RDs), a no-show fee may apply if you miss a scheduled session without providing at least 24 hours' notice. This fee is charged at the discretion of your RD and will be billed to your card on file.
12. State-Specific Disclosures
Texas Residents: To file a complaint about a physician, contact the Texas Medical Board at 1-800-201-9353 or visit https://www.tmb.state.tx.us.
Florida Residents: Practitioners are licensed and regulated by the Florida Department of Health. Consumer complaints may be filed at 1-888-419-3456.
13. Complaint Policy
All Members have the right to submit complaints or grievances about their care. To file a formal concern, email support@trymeasured.com.
14. Termination and Refunds
Measured or your provider may terminate services for non-payment, non-compliance, or if telehealth becomes clinically inappropriate. You may cancel at any time by emailing careteam@trymeasured.com; however, fees already incurred remain payable. If you meet all criteria under the six-month guarantee described in the Terms of Service, you may request a full refund during days 181–210.
