What is lean body mass?
Lean body mass is the weight of everything in the body other than fat, including muscle, organs, bone, and fluid. It is not the same as muscle mass alone, since lean mass also includes the water held inside fat tissue. On GLP-1 medications, a drop in lean mass on a body scan does not mean an equal drop in muscle.

What did the SURMOUNT-1 body-composition study find?
In a SURMOUNT-1 DXA substudy of 160 of the trial's 2,539 participants, body weight, fat mass, and lean mass fell by 21.3%, 33.9%, and 10.9% with tirzepatide by week 72, versus 5.3%, 8.2%, and 2.6% with placebo [3]. Of the weight lost, about 75% was fat mass and 25% was lean mass in both the tirzepatide and placebo groups [3].
How does that compare with lifestyle-only weight loss?
A 2026 meta-analysis of 20 trials and 15,782 participants found lean mass made up 25% to 39% of total weight lost with incretin-based drugs, compared with 26.2% with lifestyle intervention alone; the lean-mass share of weight loss was broadly comparable between the two approaches [4]. A 2024 review found muscle changes with these drugs appear to be adaptive, and factors such as older age may affect candidate selection for therapy, due to risk of sarcopenia [1] .
