What is an NAD+ injection?
NAD+ injection is a compounded product delivering the coenzyme nicotinamide adenine dinucleotide by IV or subcutaneous injection, marketed for anti-aging and wellness uses. NAD+ is not a peptide. It sits in FDA's Category 1 compounding list, and the controlled human pilot data on IV NAD+ is small and does not measure energy, cognition or aging outcomes.
What does the human data on IV NAD+ actually show?
One pilot infused 750 mg of NAD+ in saline over 6 hours into 8 healthy men aged 30 to 55, versus 3 men on saline; plasma NAD+ did not rise until after 2 hours, no adverse events occurred, and the study measured only plasma and urine metabolites, not energy, cognition or aging outcomes [1]. A 2026 review of 33 human studies from 2010 to October 2025 found "no eligible outcomes trials evaluated intravenous or intramuscular NAD+ itself for anti-aging or wellness indications" [2]. A 2026 review of IV longevity therapies, including NAD+, said they "should be regarded as an experimental intervention rather than an evidence-based dermatological or anti-aging practice" [3].
What has FDA said about NAD+ injections?
In an October 30, 2024 notice, FDA said some compounders were using food-grade NAD+ from repackagers, and that such ingredients "are not suitable for compounding sterile drugs without appropriate processing, due to the high risk of contamination with microbes and endotoxins" [4]. FDA also said it "has received adverse event reports following use of NAD+ injectable drugs, including severe chills, shaking, vomiting and fatigue," consistent with excessive endotoxin [4].
