What is a weight-loss coverage exclusion?
Coverage exclusion is a service or drug your health plan's terms say it will not pay for or cover. Unlike a prior authorization denial, which is a decision about medical necessity under the plan's rules, an exclusion means the plan does not offer that benefit at all. Some employer plans exclude GLP-1 medications prescribed for weight loss.
What makes an exclusion different from a denial?
An excluded service is one your health plan's terms say it won't pay for or cover [1]. A prior authorization denial is a decision that a drug is not medically necessary under the plan's rules, which more documentation may change; an exclusion means the plan does not offer that benefit. Even so, a denial on the grounds that the benefit isn't offered under your plan can still be appealed [2].
Do employer plans actually exclude GLP-1s for weight loss?
Some do. Blue Cross Blue Shield of Michigan stopped covering GLP-1 prescriptions for weight loss for fully insured large-group commercial members from their 2025 renewal, leaving members who continued the medication responsible for the full cost [3]. Cigna's standard drug list opens its weight-management section with the caveat that not all plans cover prescription weight-management medications, and tells members to check their plan details [4].
