Does Medicaid cover Wegovy or Zepbound?
Medicaid coverage is the extent to which a state Medicaid program pays for GLP-1 medications, which varies because weight-loss drugs are optional while diabetes drugs are mandatory. As of January 2026, 13 state Medicaid programs covered GLP-1s for obesity under fee-for-service, and four states dropped that coverage after October 2025.
Does Medicaid have to cover GLP-1 medications?
Yes, for a medically accepted indication such as type 2 diabetes, Medicaid programs must cover GLP-1 drugs like Ozempic [1]. Weight-loss drugs are different: the Medicaid statute lists "agents when used for anorexia, weight loss, or weight gain" among the drugs a state may exclude or restrict [2], so coverage of Wegovy or Zepbound for obesity depends entirely on the state a patient lives in and has changed from year to year.
Which states cover GLP-1s for weight loss, and what does it cost the program?
As of January 2026, 13 state Medicaid programs covered GLP-1s for obesity treatment under fee-for-service, while four states, California, New Hampshire, Pennsylvania, and South Carolina, eliminated that coverage after October 2025 [1]. Nationally, Medicaid's gross spending on GLP-1s rose from about $1 billion in 2019 to almost $9 billion in 2024, when GLP-1s made up over 8% of all Medicaid prescription drug spending before rebates, up from 1% in 2019 [1].
