What is the list price of a GLP-1?

List price, also called wholesale acquisition cost, is the manufacturer's price before discounts or rebates, and most insured patients pay less than it [4]. Novo Nordisk states a list price of $1,349.02 per package for the Wegovy pill, pen, and HD pen [4]. For CMS's negotiated semaglutide group (Ozempic, Rybelsus and Wegovy), the CY 2024 list price for a 30-day supply was $959 [1]. Effective January 1, 2027, CMS's negotiated Medicare maximum fair price for semaglutide, negotiated as one selected drug covering Ozempic, Rybelsus and Wegovy, is $274 for a 30-day supply, a 71% discount from that 2024 list price [1].

What do self-pay and Medicare Bridge prices look like?

Outside list price, TrumpRx.gov currently lists self-pay starting prices including the Wegovy pill at $149 a month, the Wegovy pen at $199, Ozempic at $199, and Zepbound at $299 [2]; the TrumpRx entry covers what later fills cost. Separately, eligible Medicare Part D beneficiaries can get Foundayo, Wegovy, or Zepbound (KwikPen only) for a $50 monthly copay through the Medicare GLP-1 Bridge, running July 1, 2026 through December 31, 2027 [3].
